SISC-Student Accident Coverage
SISC covers any regularly enrolled student at Taft Union High School.
Students are covered:
- While attending school on school premises
- Traveling to and from school or school sponsored activities on school-owned or operated transportation
- Participating in required school sponsored and school supervised activities
- Participating in school time and interscholastic athletics except football (See SISC Tackle Football Coverage)
- Participating in Board approved, school sponsored clubs, including travel to and from such activities if travel is on school owned or operated transportation, or if such travel is under direct school supervision
- Rule of Thumb: Insurance covers students 30 minutes before and 30 minutes after school
The coverage for students is $2500 for medical and dental expenses, SECONDARY to any private medical or dental insurance and is subject to the following limitations: 1) Chiropractic and physical therapy benefits limited to 15 visits per injury. 2) Benefits are only payable for services rendered within one year of the date of injury.
Exclusions are as follows:
- Eye glasses or contact lenses.
- Disease or illness. (This may or may not include developing conditions such as stress fractures depending on how the injury was initially handled by the student)
- Pregnancy related health issues.
- Self-inflicted injuries
- Replacement or repair of orthodontic devices (braces, retainers, etc.). It will cover the medical issues such as the injuries to the teeth themselves.
- Injuries caused when working for wages or profit. (Student workers are not covered by SISC but by TUHS workmen’s compensation)
- Injuries received while practicing or playing tackle football. (See SISC Tackle Football Coverage)
- Injuries caused by fighting (mutual fight), war, act of war, or taking part in a riot.
- Injury or illness from taking drugs unless taken on the advice of a doctor.
- Injury sustained while in or using two or three-wheeled motor vehicles.
- Injuries sustained while committing or attempting to commit a felony.
SISC Accident Coverage works using the following process.
- Accidental injuries should be reported by filling out a TUHS student accident form, or calling campus security or the athletic trainer if an emergency.
- The athletic trainer or other authorized school personnel will complete the school’s section of the claim form and gives a copy to the parent.
- It is best if the parent takes the form to the doctor with them.
- The parent or guardian completes the claim form and submits it to SISC along with itemized bills or explanations of benefits from any primary insurance coverage. Or, the parent or guardian may return the form to the athletic trainer who will fax the form into SISC.
- SISC claims examiners will accept or deny the claim and make benefit and payment determination
If a claim is questionable, it is beneficial to the claims examiners to also submit a copy of the TUHS accident report for verification of injury.
